Job Skills / Requirements

JOB SUMMARY

Responsible for direct supervision of community health workers (CHWs). Performs and monitors client assessments, care strategies and referrals for social service resources or programs.  Assesses CHW training and professional development needs and assists in securing funding sources to support CHW intervention activities.

Primary Responsibilities:

  • Provides oversight to CHWs including work schedules, assigned caseloads, equipment, supplies and project timelines.
  • Assists in recruiting, orienting, organizing training and/or professional development, and evaluating CHWs.
  • Fosters teamwork through positive leadership, relationship building and recognition of staff achievements. Work to support employee engagement and retention through promoting a positive culture that encourages high performance.
  • Consults with CHWs and multi-disciplinary healthcare teams on difficult cases.
  • Assist in developing intervention protocols, processes, and quality assurance activities for CHW interventions.
  • Develops and implements care strategies including education about services and resources.
  • Assists clients and their families in a variety of settings (e.g., clinic, community, or home setting) to ensure the maintenance of appointments and address barriers to care.
  • Documents and maintains client assessments, care plans and referrals provided.
  • Monitors client compliance with their care plan and provides motivational interviewing to support medication and treatment adherence.
  • Arranges and attends community events to connect with clients and provide clients education on case management services. 
  • Participates in continuing education programs, in-service education programs, and peer support system with other Community Health Workers.
  • Adheres to professional standards as outlined by protocols, rules, and regulations.
  • Performs other duties as assigned.

Education Requirements (Any)

High school graduate or Equivalent (GED) and completion of a medical coding certificate program

Certification Requirements (Any)

Certified CHW or completion of CHW within six months of hire
CPR Certification
Current drug and alcohol counselor certification

EDUCATION/EXPERIENCE REQUIREMENTS

  • Bachelor’s degree in social work, public health, psychology, or related field.
  • Three (3) or more years of relevant experience.
  • An equivalent combination of education and experience may be substituted (Associate degree and 5+ years of experience; Master’s degree and 1+ year of experience)
  • Possession of a valid driver’s license and a clean driving record.
  • CHW certification within six months of hire (if not already obtained prior to start date).

  • Proficient in Microsoft Office (Word, Excel and PowerPoint) and ability to quickly learn new applications.
  • Must be able to communicate with individuals of varying socio-economic backgrounds.
  • Displays ability of giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times.
