Job Skills / Requirements

Service Coordinator for our Habilitation program needed. This position provides service coordination for adults with developmental disabilities participating in our day service programs.  Duties include attending conferences for the individuals on their caseload; writing service plans (dining, high risk, and Person Centered Individual Support Plan) to provide an overall view of the client and making sure all plans are distributed appropriately; provide training to direct care staff for the clients they serve. This position also provides direct care services to the individuals we serve when needed.  Full-time, Mon-Fri 8am-4pm, Salem office.  HS diploma or equivalent and experience working with individuals with disabilities required.
